Travel Medicine

If you will be traveling to other parts of the world, a physician appointment is recommended to help you plan for a healthier trip.  Ideally, pre-travel consultation should occur at least 1 month before travel to allow time for thorough evaluation and for immunizations (some of which require more than one dose).

What is required prior to scheduling an appointment at Magers Health Center:
  • Complete and print the Travel Detail Form and up-to-date childhood/adult immunization records and fax to: 1-888-586-1348 or email: magersmedicalrecords@missouristate.edu
  • After documents are received, a staff member will contact you to set up an appointment with a primary care provider.
  • You MUST bring your insurance card or departmental coverage for travel visit appointment.   * Patient is responsible for cost incurred.
  • Please bring any additional forms (official itinerary) that need a required provider signature.
