In order to ensure compliance with the federal Health Insurance Portability and Accountability Act (HIPAA) Privacy Rules, Missouri State University and its health care facilities has developed a Notice of Privacy Practices (NPP), (requires Adobe Acrobat Reader to view - click on the link below to download).  HIPAA requires Missouri State University and its health care facilities as a covered provider, to make a good faith effort to obtain an individual's written Acknowledgement of Receipt of the Notice of Privacy Practices.

Patients who have not signed the Acknowledgement will be asked to complete the form prior to their next visit to a university health care facility, including Taylor Health and Wellness Center.  In order to expedite your waiting time, you may wish to review the NPP, print out the Acknowledgement of Receipt of the Notice of Privacy Practices (requires Adobe Acrobat Reader to view - click on the link below to download), sign it and bring it to Taylor Health and Wellness Center on your next visit or mail it to us at:
