Disclosure of PHI

The release, transfer, access, or divulging of PHI to an outside person or entity.

Disclosure relates to how you communicate protected health information to an outside person or entity. Whether the information is released orally, transferred via fax, accessed through the computer system, or divulged, discretion must be used when disclosing information. The receiving party must be authorized and have a need to know or receive the PHI.
